Insurance and Payment


  • Mindfully Integrated Health accepts the following insurance plans:

    • Medicare

    • Blue Cross-Blue Shield

    • Humana

    • Aetna

    • Cigna

    • Comp Psych

  • Please note that the insurance plans we accept may vary by provider/therapist. If you have a question about a specific provider/therapist, please speak with our staff.

  • All patients, please review the clinic policies outlined below.

  • We will be adding additional insurance carriers over the next few months, so check back often!

  • Currently we are Out-of-Network for all other insurance providers. You can check with your insurance for out-of-network benefits.

    • We do not currently accept any United Health Care plans in our outpatient office.

  • You may request a super bill to submit to your insurance provider for reimbursement of any covered out-of-network expenses.

  • As always, we offer a private pay option for all patients as outlined below.


  • Payment of charges is due at the time of service, including all deductible, copays or coinsurances as outlined by your insurance carrier.

  • We gladly accept cash, check or debit/credit card.

Financial Policies

Please read the following clinic policies as they apply to all patients:

  1. All patients utilizing insurance plans must provide insurance information at the time you schedule your first visit.

  2. Insurance benefits will be verified by MIH staff prior to your visit.

  3. All copayments and coinsurances are due at the time of service. No outstanding balances will be allowed.

  4. Release of records and/or prescriptions will be held until all balances are paid in full.

  5. If you need to cancel an appointment, you must contact the clinic not less than 24 hours prior to your scheduled appointment.

  6. Cancellation within 24 hours of an appointment or a no show appointment will result in a $75 no show fee. This will be charged to your account and must be paid prior to scheduling of a new appointment.

  7. We respect the time you are taking to address your mental health needs and ask that you respect our time as well. In order to provide you with the time necessary to provide quality care, we ask that you please have all new patient questionnaires and paperwork filled out PRIOR to arriving for your first visit.

  8. MIH staff will make every effort to contact you regarding your incomplete intake paperwork but reserves the right to cancel any appoint in which the questionnaire has not been completed as of the day of the appointment.

  9. Any appointment cancelled due to incomplete paperwork will result in the rescheduling of your appointment and a $50 rescheduling fee will be charged.
